What makes an erection happen?
An erection is a vascular event with the nervous system in charge. Arousal triggers nerves to release signalling molecules (including nitric oxide) that relax the blood vessels of the penis. Blood flows in, the veins that normally drain it are compressed, and pressure builds.
That chain depends on healthy blood vessels, working nerves, the right hormones (including testosterone) and a mind that is not flooded with stress. Adrenaline does the opposite job: it tightens blood vessels and switches the whole process off. A weak link anywhere in the chain can cause erectile dysfunction (ED), which is why finding the cause matters so much. What causes erectile dysfunction?

The pattern gives doctors real clues. ED that appears suddenly, varies with the situation and leaves your morning erections intact usually has a psychological driver (doctors call this psychogenic). ED that creeps in over months, shows up every time and takes the morning erections with it points to a physical cause (organic), most often vascular.
Why do doctors treat ED as an early heart warning?
This is the part many men have never been told. The arteries of the penis are narrow, roughly one to two millimetres across, while the heart's own arteries are three to four. When arteries begin to stiffen and narrow, the smallest pipes show it first. That is why new, persistent ED in midlife can be one of the earliest signs of cardiovascular disease, and research suggests it can precede heart symptoms by several years.

Why does erectile dysfunction flare up while travelling?
Erections rely on the calm, rest-and-digest side of the nervous system. Travel supplies the opposite: jet lag and broken sleep, more alcohol than usual, heat and dehydration, and the quiet pressure of a honeymoon, a new partner or a trip you saved two years for.
Then the loop starts. One difficult night leads to worry about the next, and worry is exactly the state that blocks the reflex. Many men begin watching themselves during sex instead of being present, which reliably makes things worse.
What alcohol, heat and late nights do
Alcohol blunts the arousal reflex on the night and wrecks the deep sleep in which most of your testosterone is produced, so two heavy evenings in a row have a longer tail than men expect. The conversation nobody wants to start with their partner
Performance pressure is rarely a solo problem. The loop runs between two people, so it is easier to break with two people.
One sentence normally does it, said outside the bedroom and without a preamble: I have been stressed and drinking more than usual, this has been happening, it is nothing to do with you, and I am going to get it looked at. Most partners are relieved to be told anything at all.

Why not just buy pills over the counter?
Around the tourist strips you will see the offer everywhere: blue diamond-shaped tablets kept in a drawer under the counter of a souvenir shop, sachets of "herbal honey" or strong-man coffee stacked beside the till in Kuta and Legian, a tout outside a bar who knows a guy. Genuine ED medicines are prescription-only in Indonesia, so anything sold this way is unregulated at best. BPOM, Indonesia's food and drug authority, has issued repeated public warnings about traditional and herbal potency products found to contain undeclared prescription drug ingredients, which is the whole reason the sachets work at all when they do.
The World Health Organization has estimated that around one in ten medical products in low- and middle-income countries is substandard or falsified, and ED medicines are among the most commonly faked in the world. Analyses of seized products have turned up too much active ingredient, too little, none at all, or contaminants never meant to go anywhere near a human.
The tells are easy once you know them. Nobody asks for a resep. There is no BPOM registration number on the box. The price is a fraction of the pharmacy price. The packaging carries text in a language nobody in the shop can read, and it is sitting next to the sunglasses. A real transaction looks different: a licensed apotek takes the prescription, records it, hands over a box with a BPOM number printed on it, and a pharmacist tells you the dose and what not to combine it with. If none of that happens, whatever is in the tablet is anyone's guess.
What if you already take medicine, or your heart is a question mark?
Genuine ED tablets are unsafe alongside some heart medicines, in particular the nitrate medicines used for chest pain, whether they come as tablets, a spray or a patch. Together they can cause a severe, sudden drop in blood pressure.
The small bottles sold as poppers in bars and clubs act on the body the same way nitrates do. Same chemistry, same danger. Taking one of those on a night when you have also taken an ED tablet can drop your blood pressure far enough to make you faint, and occasionally much worse than that. Nobody is going to lecture you about it. Say it anyway, so the advice you get fits the nights you are having.
Some blood pressure medicines and some tablets prescribed for an enlarged prostate need timing or dose care alongside ED treatment rather than avoidance. What happens in the consultation?
Less than men fear, and it is over quickly. The doctor asks when it started and whether it came on suddenly or crept up, whether it happens every time or only in some situations, whether you still wake with erections, how much you drink in a normal week and how much you have been drinking here, how you are sleeping, how your mood is, what medicines and supplements you take, and whether you have had pelvic surgery, a hernia repair or an injury.
There is no physical examination on a video call. Nothing to show, no camera moment, no gown. If an examination were needed, the doctor says so and arranges it in person, and for most men it is not needed at all. The call is on a private link, the notes stay in your record, and any delivery arrives in plain packaging with nothing on the outside naming what is inside.
If the answers point at a physical cause, testing follows. Blood pressure, blood sugar and cholesterol come from one draw. - Lifestyle: regular exercise, less alcohol, stopping smoking and better sleep measurably improve vascular ED, and unlike a tablet they work on the artery rather than on the evening.
- Treating what is underneath: the blood pressure, blood sugar or hormone problem that was quietly driving it.
- Psychological support where anxiety, low mood or relationship strain is doing the work. Sometimes two honest conversations, sometimes more than that.
- Prescription medication, where appropriate: modern tablet treatments improve blood flow on arousal and help many men, but outcomes vary, they are not suitable for everyone, and they do nothing for desire. - Chest tightness, breathlessness or calf pain that comes on with exertion and eases with rest, alongside ED: arrange a cardiovascular check this week, not this year.
- Sudden ED after an injury to the pelvis, groin or spine.

- An erection that will not go down after four hours (doctors call this priapism). Medical emergency. Go to a hospital, do not wait it out.
